To register as an individual or for everyone in your household, you will need you complete this NHS online registration form by clicking here. Alternatively, you can visit the Practice and ask for a new registration pack.
All new patients will also need to complete a new patient health questionnaire giving details of your important past medical history, medication you are taking, allergies etc. – medications will not be issued unless a new patient medical has been completed.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Once registered, you will also be able to apply for online access to do this. Please see our section on application for online access. Alternatively, you can complete the application for online access and bring this to the practice with two forms of identification, one of which will need to be photographic.
As well as permanent registrations, we can also register people if they are staying in the practice area on a temporary basis – either up to 15 days, or over 15 days and up to 3 months. You will need to provide the name and full address of your own surgery.
All practices are required to provide their patients (including children) with a named GP who will have overall responsibility for the care and support that the surgery provides to them. Your named GP at Kings Lane is Dr Philip Bacon. You may of course continue to see any GP in the practice. Please inform any family members who are also registered at this practice.
