Please ensure that you enter your current, up-to-date contact details. PATCHS can be used for requesting all appointments with the clinical staff at the surgery and requesting sick notes.

Please do not use PATCHS to request medication. Medication requests can be made by either bringing the prescription request into the practice, posting the request to the practice. Alternatively, you can request medication through downloading the NHS app or registering for Patient Access. Please ask one of the receptionists for more information on registering for Patient Access.
